We are overwhelmed with the number of deserving applicants who showed interest in this year’s Second Chance program. We are so thankful to each and every applicant who shared their story with our office and opened our eyes to the enormous amount of need in our community for quality oral surgical care. We are excited to see this program open new doors for this practice to connect with the local community and look forward to continuing to develop programs, like Second Chance, to meet their needs.

Jeff Tells Us About His Dental Health:
“My dental health is very poor. I have no teeth in the back on the bottom or top. Most of what I have left are broken or very weak. I have large holes in my teeth where huge fillings have fallen out causing instability in my teeth to an even greater degree. Growing up the oldest of five children, there was never any emphasis on dental care by our parents. I had never even been seen by a dentist until I entered the Marine Corps when I was 19 years old. Up until that time my teeth were healthy, I assume, never having issues with pain or visible decay. However, since becoming older and teeth weaker, the onset of problems became a major concern which have now gotten to the point where they can no longer be saved. My recent situation stems from an auto accident that happened while in Atlanta over 16 years ago. I was broadsided by a drunk driver while in a company van going to get my hair cut for my upcoming National Guard drill that weekend. The impact tore open my upper lip requiring a plastic surgeon to repair the damage. About a year or so later, I discovered that the trauma to my upper lip caused vertical fractures to my two front teeth which eventually broke one in half. The insurance company that the workman’s comp claim was filed through would not pay for the teeth to be repaired stating the damage was not reported initially from the incident and that too much time had lapsed in requesting compensation for the injury. Although I had two daughters in braces at the time and could not afford any more monthly expenses, I waited until I could afford to spend the $4,000.00 to have crowns affixed to my front teeth. That period of time was the most embarrassing and self-conscious of my life. I did not want to smile, although that is what I love to do the most and couldn’t. Simply put, my teeth are affecting my health!!”
Why Restoring Jeff’s Smile is Important to Him:
“In my profession as a police officer, I come into contact with the public more than people realize. I not only make contact through the everyday calls I am assigned to, people see me wherever I go in uniform. Everyone recognizes the uniform and looks at me no matter the situation or location. There are a number of people, thankfully, that still appreciate the job we do and will go out of their way to say “hello or thank you,” that is why I do this job. Not for the money, glory, power or prestige, simply because they feel safer knowing someone is watching over them so they can live life without constantly looking over their shoulder. It’s the same feeling I got from my 23-year career in the military, the citizens could feel comfortable sleeping, traveling, and living their lives knowing they are safe and secure. I enjoy these encounters with the citizens and want to continue to smile a big smile back at them and say “you’re quite welcome, my pleasure”. However, now I am not wanting to smile due to the huge gap in my smile and only grin and nod, which to me is NOT genuine and sincere. I am a servant of the people of my county, and they should not fear me and feel as if I am not approachable because I cannot greet them in a manner that they should be because I am self-conscious. If there is one thing a police officer should portray to the public, it is confidence, that they can feel safe knowing you are a confident and competent professional. I want to do that with a genuine smile!!”
What Makes Him a Great Second Chance Recipient:
“After listening to Dr. Falender’s interview on the radio about the Second Chance Program, I began to think about how this opportunity would not only affect my life dramatically but possibly countless others as well. I am a smoker and have been since I was 16 years old. I have never been a healthy eater, especially lately since my teeth are a wreck. I don’t really know the full scope of how this opportunity will affect my life, but do know it will greatly affect my health! And that is the most important thing to me as I approach my 55th birthday. You see, just five weeks ago I was laying in ICU in South Carolina twelve hours away being no longer alive from an infection driving toward my chest. Doctors worked quickly to combat the rush of the infection to my chest, and the very concerned look on my wife and daughter’s faces as they gathered around me made me realize that changes had to be made. My wife and I have just relocated back to Indiana in December 2014 to be closer to our daughters and grandchildren. I have left the sheriff department in South Carolina and returned to a position as a jail officer in Hendricks County while I apply for and go through the lengthy process of being hired as a police officer here in Central Indiana. I am currently in the process for hire at Indianapolis Public Schools as a police officer and possibly Captain of Police there. The Second Chance program would have the biggest impact getting back to being me and smiling that big smile the day I am offered the opportunity to impact lives in a positive way again. I have greatly reduced the number of cigarettes I smoke in a day but want to get away from them completely. I want to get healthier and be there for those who will rely on me for many years to come: my wife, daughters, and grandchildren. They are my LIFE, and I want to smile at every small and large moment that comes along in their lives. Pictures of the family will last generations. I want to be smiling in them, showing the joy they bring me and that I love to smile BECAUSE of them. I want them to remember my smile for years to come, not a grin. I have never been a person that was comfortable in taking anything for free or that I did not work for but, given this second chance at the opportunity to smile big again, I promise I will never stop!!”

Official Terms & Conditions
Second Chance is a program offered by Indianapolis Oral Surgery & Dental Implant Center in an effort to “give back” to the community, one person at a time. It is not a contest or a game, but instead an opportunity for one person to have the ability to improve their smile, boost their self-confidence, and get a second chance on life. It starts with a smile…..NO PURCHASE OF SERVICES IS NECESSARY TO BE CONSIDERED FOR THE SECOND CHANCE PROGRAM (“Program”). A purchase, or prior purchase, will not increase your odds of being selected. All federal, state, local and municipal laws and regulations apply. Void where prohibited.
Eligibility. The Second Chance program is open to individuals 18 years or older at the time of application and is only open to legal residents of Indianapolis and surrounding areas. Employees of Indianapolis Oral Surgery & Dental Implant Center or co-sponsors, affiliates and partners, and their immediate family members of, and any persons domiciled with such employees, are not eligible.
Sponsors and co-sponsors. The Second Chance program is sponsored by Indianapolis Oral Surgery & Dental Implant Center and the participating co-sponsors. Selection of a participant and all other aspects of the Program shall be final and binding on entrant in all respects. Neither Indianapolis Oral Surgery & Dental Implant Center nor Program co-sponsors, affiliates, nor partners will be responsible for typographical, printing or other inadvertent errors in these Terms & Conditions or in other materials relating to the Program. Application. The Program begins at 12:01 a.m. (EST) on November 3, 2014, and all entries must be received by 11:59 p.m. (P.D.T.) on January 31, 2015. Individuals may enter this Program by filling out the Official Application Form located on this webpage. Each individual can only enter once; multiple entries will not increase the applicant’s chances of being chosen as the final participant for the Program. No one is permitted to submit an application on behalf of another individual for any reason. Any application must be submitted by entrant only and any application submitted by an individual other than the subject will void such application. In case of dispute as to identity of an entrant, such application will be declared made by the registered user of the e-mail account provided, and if selected, such registered user must be a valid entrant, and the selection will be awarded to such registered user. Registered e-mail account holder is the natural person who has been assigned an e-mail address by an e-mail provider. All online entries will be declared made by the registered user of the e-mail address. Any questions regarding the owner of an email account shall be determined by Indianapolis Oral Surgery & Dental Implant Center in its sole discretion, and Indianapolis Oral Surgery & Dental Implant Center reserves the right to disqualify any entries by persons determined to be tampering with or abusing any aspect of the Program. Internet Application: Enter by visiting the official registration page on the Second Chance program web page on www.indianapolisoralsurgery.com and access the Official Application. Form: Complete the Official Application form by following the instructions on the website. Your computer must accept cookies, or any successor or similar technology, which may be used for the purpose of application tracking. By entering, you agree to the terms of these Terms & Conditions and to receive e-mails from Indianapolis Oral Surgery & Dental Implant Center or Program co-sponsors or those directed to them. You can opt-out of the receipt of such e-mails by following the directions in any email received from Indianapolis Oral Surgery & Dental Implant Center or Program co-sponsors, their affiliates or partners.Indianapolis Oral Surgery & Dental Implant Center and Program co-sponsors, their affiliates and/or partners will not be responsible for incomplete, lost, late, misdirected or illegible applications or for failure to receive applications for any reason, including but not limited to transmission or technical failures or malfunctioning of any telephone connection, network, hardware or software. Selection and Odds. One applicant will be selected to receive a “Second Chance Makeover” (approximate retail value $20,000 – $60,000) and will be selected by a committee formed by Indianapolis Oral Surgery & Dental Implant Center from among candidates chosen through review of applications, consultations, interviews, etc. A qualified candidate must meet the Eligibility requirements defined herein. A finalist will have individual interviews with the Dr. Lawrence G. Falender and the staff of Indianapolis Oral Surgery & Dental Implant Center, and undergo initial consultations and/or examination as appropriate. The final selected recipient will be selected from the pool of applicants. Selected candidates may receive one or more of the following services: restorative dentistry, implant dentistry, oral surgery or applicable services. Odds of selection depend on the number of eligible entries received.
Notification and Acceptance. Finalists will be notified by phone, email, and/or U.S. mail. If a potential candidate does not respond to the notification (with all requested information) within ten (10) days, such potential candidate will be disqualified and an alternate selected. Forfeiture of selection for the Program and the selection of an alternate (provided sufficient eligible applications are received) selection may result from any of the following: applicant: (1) fails to respond to a notification (with all requested information) within the required time; (2) is sent an email notification that is returned as undeliverable after three (3) attempts; (3) fails to complete and return required Affidavit of Eligibility and Liability and Publicity Release (and any other documents, as requested) within the required time; (4) is sent a notification/documentation that is returned as undeliverable for any reason; (5) is deemed ineligible or cannot be notified or contacted; (6) is unable to provide satisfactory proof that he/she is the authorized account holder of the email address (7) is not available to have procedures performed on the dates and times specified by Indianapolis Oral Surgery & Dental Implant Center; or (8) fails to respond to any other required time periods or is otherwise not in compliance with the Terms & Conditions; Applicant is solely responsible for reporting and payment of any taxes, if applicable. Selected applicant will be solely responsible for all necessary travel and other expenses incurred in connection with accepting the treatment.Applicants will be required to sign and return an Affidavit of Eligibility and a Liability and Publicity Release which must be returned within ten (10) days following the date of first attempted notification. If selected, participation in Program not claimed by eligible applicants in accordance with these Terms and Conditions will void application and participation, resulting in full forfeiture of participation in the Program. Participation. By participating, applicants agree to be bound by these Official Terms and Conditions, and the decisions of the committee. Indianapolis Oral Surgery & Dental Implant Center reserves the right to disqualify persons found tampering with or otherwise abusing any aspect of this Program as solely determined by Indianapolis Oral Surgery & Dental Implant Center. In the event the Program is compromised by a virus, non-authorized human intervention, tampering or other causes beyond the reasonable control of Indianapolis Oral Surgery & Dental Implant Center which corrupts or impairs the administration, security, fairness or proper operation of the Program, Indianapolis Oral Surgery & Dental Implant Center reserves the right in its sole discretion to suspend, modify or terminate the Program. Should the Program be terminated prior to the stated application end date, Indianapolis Oral Surgery & Dental Implant Center reserves the right to select an applicant based on the entries received before the termination date.
Copyright. By entering the Program, each applicant grants to Indianapolis Oral Surgery & Dental Implant Center and Program co-sponsors an exclusive, royalty-free and irrevocable right and license to publish, print, edit or otherwise use the applicant’s submitted application, in whole or in part, for any purpose and in any manner or media now known or hereinafter developed (including, without limitation, the Internet) throughout the world in perpetuity, and to license others to do so, all without limitation or further compensation. If his/her application is selected to proceed to a group and/or individual interview, applicant will sign any additional license and release that Indianapolis Oral Surgery & Dental Implant Center and Program co-sponsors may require to provide that Indianapolis Oral Surgery & Dental Implant Center and the Program co-sponsors and any other promotional sponsors, and each of their respective affiliated companies, directors, officers, employees, representatives, partners and agents, may use applicant’s name, biographical information, address, picture/photograph/video likeness (including before, during and after photographs/video) and/or voice, for advertising and promotional purposes without further consideration to applicant in connection with applicant’s submission to and participation in the Program.
Limitations of Liability. By entering the Program, applicant agrees that (1) any and all disputes, claims, and causes of action arising out of or in connection with the Program, or any services provided, shall be resolved individually without resort to any form of class action, and applicant waives his or her right to a jury trial for such disputes, claims, and causes of action; (2) any claims, judgments and awards shall be limited to actual out-of-pocket costs incurred by applicant, including costs associated with entering the Program, but in no event attorneys’ fees; and (3) under no circumstances will any applicant be permitted to obtain any award for, and applicant hereby waives all rights to claim punitive, incidental or consequential damages and any and all rights to have damages multiplied or otherwise increased and any other damages, other than damages for actual out-of-pocket expenses. BY ACCEPTING SERVICES, THE APPLICANT AGREES THAT INDIANAPOLIS ORAL SURGERY & DENTAL IMPLANT PROGRAM, PROGRAM CO-SPONSORS AND THEIR OFFICERS, DIRECTORS, EMPLOYEES, REPRESENTATIVES AND AGENTS, WILL HAVE NO LIABILITY WHATSOEVER FOR, AND WILL BE HELD HARMLESS BY APPLICANT FOR ANY LIABILITY FOR ANY INJURY, LOSS OR DAMAGES OF ANY KIND TO PERSONS, INCLUDING DEATH, AND PROPERTY, DUE IN WHOLE OR IN PART, DIRECTLY OR INDIRECTLY, FROM THE ACCEPTANCE, POSSESSION, USE OR MISUSE OF THE SERVICES PROVIDED HEREUNDER OR PARTICIPATION IN THIS PROGRAM OR PARTICIPATION IN ANY PROGRAM OR RELATED ACTIVITY.
Construction. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Terms and Conditions, or the rights and obligations of any applicant and Indianapolis Oral Surgery & Dental Implant Center or co-sponsors, shall be governed by, and construed in accordance with the laws of the state of Indiana, without giving effect to any choice of law or conflict of law rules or provisions. The invalidity or unenforceability of any provision of these terms and conditions shall not affect the validity or enforceability of any other provision. If any such provision is determined to be invalid or otherwise unenforceable, these rules shall be construed in accordance with their terms as if the valid or enforceable provision was not contained therein.
General Release. By submitting an application to the Program, applicants release Indianapolis Oral Surgery & Dental Implant Center, Program co-sponsors and any other promotional sponsors, and each of their respective affiliated companies, directors, officers, employees, representatives, partners and agents from any liability whatsoever for any claims, costs, injuries, losses or damages of any kind arising out of or in connection with the Program or with the acceptance, possession or use of any services relating to the Program (including, without limitation, claims, costs, injuries, losses or damages related to personal injuries, death, damage to, loss or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light).
Notice. Any claim, notice or other written document to be served on the Program, shall be deemed served and delivered when mailed to the Program by certified mail, return receipt requested, to Indianapolis Oral Surgery & Dental Implant Center 1320 N. Post Road Indianapolis, IN 46219.
Choice of Law, Jurisdiction, and Venue. Any litigation that involves the Program shall be governed by the law of the state of Indiana. All participants in the Program consent to the jurisdiction and venue for any such litigation concerning the Program to be brought solely in the federal or state court presiding in Indianapolis, Indiana.
